SPYB Winter Rec Registration Form 26′-27′

I/We _________________________ the parents of the above-named registrant(s) do hereby grant my/our permission for my/our son(s) or daughter(s) to participate in all activities of South Plymouth Youth Basketball. I/We do know that participation in basketball may result in serious injury and protective equipment does not prevent injuries to all players.

I/We do hereby authorize treatment under the direction of any licensed physician, for the above-named minor in the event of a medical emergency which, in the opinion of the attending physician, may endanger his or her life, cause disfigurement, physical impairment, or undue discomfort if delayed.

This authority is granted only after reasonable effort has been made to reach me by phone at the number listed above. The undersigned assumes the responsibility for any cost connected with such treatment and does hereby release, waive, absolve, indemnify, and agree to hold harmless the SPYB, its directors, sponsors, organizers, participants, and persons transporting my/our child, whether the result of negligence or for any other cause, except to the extent of and in the amount covered by liability insurance from any liability thereof.

This release form is completed and signed of my own free will with the purpose of authorizing medical treatment under emergency circumstances in my absence.

I/We, the parent(s) or legal guardian(s) of the above-named registrant(s), hereby grant South Plymouth Youth Basketball (SPYB) permission to photograph and/or record video of my/our child(ren) during league-related activities including games, practices, clinics, and events. These images or recordings may be used by SPYB for league-related purposes such as website content, social media, email communications, or promotional materials intended to highlight and support youth basketball programs within the SPYB community.

🏀 Mandatory Winter Rec Evaluations

South Elementary Gym • Grades 3–8
3rd/4th: Mon. Nov. 9 • 5:15–7:15 PM
7th/8th: Tue. Nov. 10 • 5:15–7:15 PM
5th/6th: Thu. Nov. 12 • 5:15–7:15 PM

Players complete a 15–20 minute evaluation, first come, first served, in groups of 10. Once one group finishes, the next group will begin, continuing until all players have been evaluated.

K–2nd Grade: There are no evaluations for K–2nd Grade. Teams will be created by the league and announced by November 14.

Winter Travel tryout dates, times and Tryout Sign-Up information are now posted. After completing your Winter Rec registration, visit the SouthBBall.com homepage for full Winter Travel Tryout information.

Reminder: Players interested in Winter Travel must also be registered for Winter Rec. The current Travel sign-up is for tryouts only. Travel team registration information will be provided after Winter Travel tryouts have concluded and teams have been selected.
